Physicals at Center for Sports Medicine May 1, 2024
If your current 6th ,7th or 8th grade student is interested in playing sports for the 2024-2025 school year and would need a physical, HMS will be taking athletes over to the Center for Sports Medicine to have their physicals done on Wednesday, May 1. The cost is $25. Please make checks payable to HMS. Please have your child pick up a physical form, permission slip, and athletic packet in the HMS front office.
All students must have a current and completed Athletic Packet Signature Form, Heritage Middle School Athletics Medical Card, and Physical Form on file before they can participate in 2024-2025 tryouts, workouts, practices, etc.

Students must be enrolled at HMS to tryout and participate in athletics.
